Support Worker

An exciting opportunity has arisen for a Support Worker position within our Older People's acute services to support our service users with all aspects of their care including activities and personal care. This role focusses on the direct needs of service users by delivering individual treatment plans across boundaries of care. Hello! To provide support, give time to service users within the unit and the community and thus promote recovery. To provide holistic care, taking into account psychosocial and physical healthcare needs of the service user group. To assist Care Co-ordinator or key worker and service user to assess, plan, implement and evaluate individual care plans and undertake other assessments as required.
