Job SummaryWorking with the guidance of a clinician and strictly in accordance with policies, procedures, guidelines and protocols. The healthcare assistant will have a specific focus on the delivery of Annual Health Checks for those with Severe Mental Illness - SMI. Main Duties and Responsibilities: Annual Health Checks for patients with SMI Other NHS Health Checks Measuring and recording: Blood pressure Pulse rate Height and weight Body mass index Point Of Care Testing Blood Tests Cleaning and de-sanitising and maintenance of equipment Supplies, equipment re-stocking and stock rotation Preparing and maintaining environments and equipment before, during and after patient care interventions. Assisting in the assessment and surveillance of patients health and well-being Raise awareness of health and well-being and how it can be promoted Assisting with the collection and collation of data related to health and well-being Completing administrative tasks in line with the role including contacting patients over the phone to complete health checks Any other duties deemed appropriate for the role by the responsible clinician Administration Record information and activities undertaken with patients and clinicians in an accurate and timely fashion using manual or computer systems as appropriate Record information accurately and in a timely fashion using Excel spreadsheets Maintain confidentiality with regards to information relating to patients, relatives, staff and practice. Other duties Practice in accordance with agreed standards of care and always ensure own actions are consistent with clinical governance systems Alert other team members to issues of quality and risk in the care of patients Understand and follow the Alliance policies and procedures. The duties of the post may be reviewed and, if appropriate, changed by discussion with the post holder to ensure the provision of effective and efficient services.
