Frailty GP

We are seeking to appoint a number of GPs with an interest in frailty to support the delivery of various innovative Frailty Services as part of the North Yorkshire Community Frailty Hub . The postholder will undertake a variety of duties, primarily providing clinical advice and support to system providers regarding vulnerable patients in the community as part of a multidisciplinary team.
