Job summary

Role: Advanced Clinical Practitioner (MSc) or Credentialled You will work with our friendly, supportive, ever-expanding team of clinicians here at FCMS Urgent and Emergency Care, Yorkshire. To deliver high quality, NHS care to the people of Doncaster, 24 hours a day, 7 days a week. The post holder will be expected to work across all areas including but not limited to the Urgent Treatment Centre (UTC), the Same Day Health Centre (SDHC), the ECP home visiting service and the OOH triage services. The services we run allow patients to access same day health services and support NHS services across Doncaster, serving a population exceeding 300,000. Practice under senior supervision as a practitioner, participating in the care of and undertaking assessment, diagnosis, treatment / referral and discharge of patients presenting with minor injury and illness within agreed parameters Provide advanced clinical skills and advice demonstrating a sound understanding of the issues related to identification, assessment, diagnosis, treatment and management of a client group whilst working autonomously Provide advice, information, knowledge and support to staff. Identify local, demographic, cultural and environmental factors that influence the health and quality of life. Utilise non-medical prescribing qualification adhering with local formularies, policies and Clinical Governance to provide appropriate non-medical prescribing support for the organisation in adherence to Clinical Governance frameworks and in adherence to the agreed policy. Undertake annual audit of prescribing practices. Ensure that child protection and vulnerable adult legislation is adhered to within their own practice and the service. Assist with the development and evaluation of policies/guidelines/protocols for advanced and specialist clinical practice Continually review and evaluate own role and clinical practice informing the annual review process with the Clinical Lead. Plan and manage their own workload and the workload of the service, in a flexible manner, ensuring that service provision meets the needs of patients. Promote a positive relationship with all stakeholders within the service. Contribute to the clinical governance process, including adverse incident reporting/investigation and the management of complaints, as appropriate. Be involved in peer review of own practice and that of others. Be flexible to support the need of the service, across both sites and the home visiting service. Professional The post holder will: Role model standards of care and behaviour through clinical practice. Analyse relevant national policy, within their area of expertise and advise on the impact for service, leading the implementation of changes and evaluation of processes. Maintain professional registration and practice through CPD. Maintain a professional portfolio to demonstrate competency in line with the advanced practice framework. Maintain personal standards of conduct and behaviour consistent within FCMS and their independent governing bodys guidelines. Ensure appropriate use of resources to meet service needs. Recognise and act as an advocate for patients, carers, the service and the organisation. Contribute to change as required and ensure resolution of conflict that may occur. Promote workforce integrity (empower professional accountability within the clinical team, ensure accreditation process followed, complaints management, competency assessment, ensuring mandatory training requirements fulfilled, robust clinical induction) Participate in regular clinical supervision. Ensure CAS & NICE alerts are followed accordingly Maintain a sound knowledge of NHS policy and NICE guidance in relation to Urgent Care. Participate and contribute to our monthly Clinical Supervision session to continuously support with learning and development of safer care teams, processes and services Non-Medical Prescribing You will need to possess prescribing capacity and must have a current registration with your professional body that includes V300 qualification. You must only prescribe from an agreed Personal Formulary which reflects your competency and experience and is aligned to up to date evidence / best practice guidance. You will be expected to review and develop your Personal Formulary on an annual basis. Go-getting: We are intuitive to changing needs and respond quickly which we do with energy, ideas, and positivity Key areas of responsibility will include but are not limited to: Clinical Education and training The post holder will: Hold an MSc in Advanced Clinical Practice or Equivalent with Level 7 Qualifications and be Credentialed. Work autonomously. Be responsible for identifying their own development needs in line with service requirements and a personal development plan. Develop new ways to improve service and lead with changes. Provide clinical supervision both within the organisation to support the development of individuals and the service. Ensure ACP clinical supervision training is completed Ensure the clinical environment is conducive to supporting the education and learning of all staff and students Provide educational and training opportunities to students to ensure placements satisfy the relevant elements of their learning agreement Contribute to clinical governance outcomes. Understand and apply professional and national guidelines on confidentiality. Be fully involved in the progress and development of clinicians and provide constructive and relevant feedback. Be a Designated Prescribing Practitioner for non-medical prescribing students Offer face to face clinical supervision Research and development The post holder will: Evaluate clinical practise through audit of service and develop and manage strategies to address any shortfalls Undertake audits as required by the service working with the clinical manager Demonstrate knowledge of current research in all aspects of their work and advise others on the implementation of relevant research findings Initiate and/or become involved in research projects in response to identified needs Disseminate results/research/ audit findings via local teams and/or national/international publications Implement research findings appropriate to practice Promote the shared objectives of the multidisciplinary team by working closely to ensure that best practice is achieved. Communication and working relationships The post holder will: Communicates effectively with other team members Communicates effectively with patients and carers Communicates effectively with other healthcare professionals Take all appropriate steps to minimise conflict and deescalate situations Clearly document all contacts with a patient and refer on as appropriate Health and safety The post holder will: Comply with safety policies, procedures and guidelines for self and others Comply with the Organisations Business Continuity Plan Refrain from the wilful misuse or interference with equipment provided in the interest of Health and Safety Report as soon as is practically possible any hazards or defects Report as soon as practically possible accidents or untoward incidents and ensure appropriate incident reporting documentation is completed Be actively involved as an appropriate departmental representative Making effective use of training to maintain knowledge and skills Physical effort and working conditions The post holder will: Be responsible to ensure all aspects of the Manual Handling guidelines are followed, providing appropriate risk assessments Be responsible to manage clinical care following required Policies and Procedures pertaining to bodily fluids, infection control and COSHH regulations resulting in frequent exposure to highly unpleasant working conditions Be responsible to manage challenging situations and people with some exposure to hazards, e.g. violent and abusive patients. Child abuse, critically ill patients Manage own time effectively within variable shift patterns Any other duties considered relevant to the post and commensurate with the post holders grade and as agreed by their manager.
